The Oak PTA hopes you will find this Back-to-School (BTS) Checklist useful to keep track of the items you need to turn in at the beginning of the year.
Please set aside 10-15 minutes to complete all the BTS forms. If you do not complete the BTS forms and receive an order summary, the information will not be retained and you will need to re-enter all your information when you return to the site. You can complete the other checklist items at your convenience but please make note of the relevant deadlines.
